Chronological Reports
After organization and pagination of the records, our legal nurse
consultants will complete a comprehensive review of the records
and draft a chronological reconstruction of the significant medical
events. The length and detail of the chronology depends upon the
volume of records, the complexity of the medical issues as well
as the type of case, i.e. personal injury or medical malpractice.
CaseMap, a case management software, allows the chronological
evidence to be sorted by date and time, record page number, author
or facility. The evidence can be further sorted by diagnosis,
body part, procedure, or medication. CaseMap facilitates
analysis of the evidence, early identification of primary issues
and earmarks questions in need of additional discovery.
Additionally, we will conduct preliminary research of medical
literature to define pertinent mechanisms of injury, procedures,
treatments, or disease processes.
The Chronological Report reveals patterns, trends, and chains
of events that were previously unclear. The report makes order
out of chaos, clearly revealing the story told by the medical
evidence.

TimeMap
The TimeMap program creates a graphic depiction of the key
evidence identified in the CaseMap chronology. TimeMap
is designed to visually enhance the facts and clarify events that
occurred both before and after the alleged incident. By color
coding the facts and simplifying complex concepts, the observer
is able to recognize key evidence at a glance. One color may represent
normal values or conditions, while another color may reflect abnormal
values or conditions. A bright red color may indicate timing of
the alleged incident. This tool has become a mainstay for our
clients at deposition and mediation.

Evaluation of Medical Merit Report
The Evaluation of Medical Merit Report (EMMR) contrasts and compares
the evidence in the medical records with the known standard of
care and defines the case merits. This report becomes the information-backbone
of the case and:
- Defines negligent acts
- Develops causation theories
- Substantiates or denies damages
- Names potential defendants and material witnesses
- Recommends additional discovery
- Is a reference to discuss facts with potential
expert witnesses
- Is a resource for deposition and mediation
or trial preparation
Comprehensive research of the medical literature
is complied regarding the standard of care, mechanism of injury,
treatments, procedures, disease processes, and prognosis. This
research allows the attorney to develop a working knowledge of
the medicine specific to the case at hand. The EMMR contains detailed
allegations annotated with the compiled medical literature, as
well as candid discussions regarding weaknesses in the case and
anticipated defense perspectives.
The report lays the foundation for case-theme
development. As the discovery process continues, a single "Ultimate
Issue" will emerge. The attorney can then develop focused
arguments communicating the case merits to the jury.
