In litigation that is based upon medical records,
comprehensive medical information should be obtained. The first
step of this process involves ascertaining the patient's complete
medical history. Our "Medical History Questionnaire©"
will assist in gleaning much of the salient information about
the plaintiff. The "Medical History Questionnaire©"
will act as reminder of key events.
The second step involves requesting information
from the various medical providers with an appropriately signed
and dated patient's authorization for release of information.
We also recommend a request letter accompany the release as well
as an affidavit to be signed by the custodian of the records.
The affidavit will certify that the copies are a true and complete
reproduction of the patient's medical records. Records may be
obtained the traditional way - through the mail, however, there
are now several companies offering to obtain records for the attorney
via the Internet.
The third step is to organize, paginate and index
the comprehensive records for ease in key document location. "The
